HI,SNOOPY,

i have to confess ! i cheated from you questionnaire to compile this form.i am guilty!! 

ANIMALS ONLINE: 

What is the chief complaint (CC)? Tell as much about it as you can, including what is the worst part of it and why it's the worst. 

Has the complaint made it restless, weak, nervous, anxious, irritable, hypersensitive, affected its thirst and appetite, body temperature, and so on?

When did this problem begin? What do you think caused it?

What aggravates the CC and what brings it on? (For example, certain types of food or weather, movement, light, noise, company, talking,
heat/cold, or anything else that you can think and what makes the CC better (e.g. hot or cold, being petted, eating, lying still,music,company)?
 

At what time of the day or night is the CC the worst? Specify an hour if you can.

What symptoms can you identify that accompany the CC? E.g hunger, anger etc.

GENERAL QUESTIONS
Environment: any temperature preferences? Sun or shade? Baths?
 

HEALTH HISTORY: any serious illnesses? Treatment given? Progress? Any surgery? What and when?

Warts, cysts, Polyps, or tumors? Where were they located? How were they treated?

SENSITIVITY: any allergies? Food sensitivities? Addictions to certain food types, dislikes etc allergies to Paint? Vitamins? Herbs? Family history (if possible).  

BEHAVIOUR: any predominant characteristics e.g.lazy, restless, crying, desire to leave home, clinging, sympathy seeking, hot-tempered, jealous, highly strung,nervous,etc.

APPEARANCE: e.g. Obese,scrawny,legs bowed/weak,bloated stomach etc

please add,criticise,emend,feel free to comment!there must be something we can do to improve it.

bump for the day crew.

And I think my experience from my cat being calc carb suggests that the questionnaire might also consider some physical attributes as well.   Her obesity, short limbs and difficulty in moving around much was part of the picture.

Hi seeker.  Ginger is sitting right here, letting me know I'm a deficient mom, although she isn't being very specific about her complaint.  (OK, so it doesn't take much for me to feel guilty about the cats.)  Thanks to the aconite she was able to recover, emotionally, right away after coming back from the vets.  However, the antibiotics seem to have created a condition of very loose stools, almost diarrhea.  I repeated the mercury after the ABs, but the condition has persisted.  At the moment I'm concentrating on Bandit's megacolon condition, and frankly, it's helpful being able to tell their stools apart so easily.  So, I haven't done anything yet to try to straighten out Ginger.  Other than the very loose stools she seems to be fine.  I see my homeopath in another week, I'll try to check with her then if there is time left over.  At least with Bandit I know she's calc carb - she's the poster kitty after all.  I think the jury may still be out whether Ginger is mercury.  I think I need one of Wim's poems for that.  ;^)

Cats and dogs seem such experts at being able to induce guilt trips in their parents,don't they?

All they have to do is to sit in front of their mum/dad and stare silently and constantly  and if the person's very sensitive to the vibes,he/she will shortly start feeling guilty and anxious    

I can see you are helplessly trapped in the paws of an expert emotional blackmailer

I also get blackmailed on a daily basis so I can sympathize
